This position is primarily remote, allowing for a flexible work-life balance while focusing on client needs. You will manage schedules to conduct client visits, ensuring personalized care. You will operate within a school facility that currently serves as our primary workspace, providing an engaging and supportive backdrop for both staff and clients. Your work will often extend beyond standard hours; thus, occasional evening or weekend availability may be required to accommodate client consultations or urgent needs.

Key Responsibilities\n

• Perform thorough assessments of seniors’ psychosocial needs by evaluating their emotional, mental, and social factors to determine effective care strategies.

• Collaborate closely with clients and their families to create tailored care plans that align with their individual goals, preferences, and unique circumstances.

• Provide counseling and emotional support, guiding clients and family members through significant life transitions and challenges, such as health issues or loss of independence.

• Act as an advocate for seniors’ rights, helping them gain access to community resources and services that benefit their health and well-being.

• Coordinate care with interdisciplinary teams, including nursing professionals and caregivers, to ensure a holistic approach in meeting seniors’ needs.

• Conduct regular home visits to monitor clients' conditions, ensuring safety and addressing any urgent concerns related to their living environment.

• Facilitate effective communication among family members involved in the care process, resolving conflicts and promoting collaboration.

• Educate clients and their families about community resources, support groups, and programs available to enhance social engagement and overall quality of life.

• Maintain detailed and up-to-date documentation of client interactions, assessments, and care plans using electronic health record (EHR) systems.

• Stay informed of best practices in social work, remaining compliant with ethical standards and any changes to social work policies and regulations.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

The position offers a competitive salary ranging from USD 60,000 to USD 100,000 annually, depending on experience and qualifications.
