Work Environment\n

This is a remote position, allowing for great flexibility in your work schedule while serving clients based out of our community center. Your main working hours will be Monday through Friday from 8:00 AM to 4:30 PM, but occasional evening or weekend hours may be required to meet the needs of clients. Flexibility in your schedule is key to ensure consistent support and care for clients and their families.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs through interviews and collaboration with family members, considering factors such as emotional health, social support, and living conditions.

• Develop personalized care plans tailored to individual client goals, by utilizing collaborative methods with clients and their families, ensuring their preferences are prioritized.
\n
• Provide ongoing counseling and support to seniors to help them navigate personal challenges, life transitions, and complex emotional situations.
\n
• Act as an advocate for clients, connecting them with community resources and services, and empowering them to access the support they need.
\n
• Work closely with a multidisciplinary team comprising nurses, medical professionals, and caregivers to coordinate effective care strategies and interventions.
\n
• Conduct regular remote check-ins to monitor clients' conditions, addressing immediate concerns and ensuring their safety and comfort at home.
\n
• Facilitate open communication among family members to manage conflicts regarding care and support, ensuring a cohesive approach to client well-being.
\n
• Educate clients and families on available resources and programs that may enhance their social connectivity and overall quality of life.
\n
• Maintain detailed and accurate records of client assessments and interactions within our electronic health record (EHR) system to ensure compliance with regulatory standards.

• Stay informed on the latest developments in social work practices to continuously provide services that meet or exceed ethical and professional standards.

This position focuses primarily on working with elderly clients, many of whom may be facing age-related health challenges, emotional difficulties, or social isolation. Qualifications\n

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
