Key Responsibilities\n

As a Licensed Clinical Social Worker with Bridge Way Reflections Therapy, your primary responsibilities will include:

• Providing high-quality counseling and emotional support to patients and their families, assisting them in addressing adjustment issues and coping strategies in the context of recovery.

• Collaborating closely with rehabilitation professionals to develop tailored treatment plans that reflect each patient’s unique psychosocial needs and rehabilitation goals.

• Assisting patients in identifying and accessing community resources, including support groups, vocational rehabilitation services, and housing assistance, to promote overall well-being and independence.

• Engaging in proactive discharge planning to facilitate smooth transitions for patients moving back to their homes or other care settings.

Qualifications\n
