Key Responsibilities\n

• Engage directly with clients in various settings, both remotely and in community outreach initiatives, providing critical assessments and therapeutic interventions.

• Design and implement individualized care plans that direct comprehensive treatment tailored to the unique needs of each hospice client.

• Conduct brief, evidence-based screenings to facilitate appropriate referrals for additional mental health support when required.

• Carry a caseload that includes assessments, individual and family therapy sessions, and coordinated care efforts among a spectrum of service providers.

• Work collaboratively with a team to ensure best practices in mental health treatment, emphasizing choice and empowerment for clients.

• Support clients during crises with timely intervention strategies and follow-up services.

• Ensure compliance with all ethical guidelines, professional standards, and organizational policies in conducting assessments and providing therapeutic services.

• Contribute to quality improvement initiatives and actively participate in agency committees aimed at enhancing service delivery.

• Respond to on-call needs during designated after-hours shifts as part of team coverage responsibilities.

• Prepare and submit comprehensive reports, tracking client progress and service outcomes to ensure transparent communication within the team.

This position focuses on clients experiencing severe emotional disorders and major mental illnesses within the hospice care context. Qualifications\n

The ideal candidate will possess the following credentials and skills:

• A Bachelor's degree in Social Work (BSW) from an accredited institution is required; a Master's degree (MSW) is preferred.

Compensation and Benefits\n

The salary range for this position is between USD 70,000 and 85,000 per year, commensurate with experience and qualifications.
