Key Responsibilities Conduct comprehensive mental health assessments that take into account the interplay between mental and medical health conditions. Develop individualized treatment plans that outline evidence-based therapeutic modalities suitable for addressing the diverse needs of your patients. Facilitate individual therapy and support group sessions, utilizing tools and methodologies grounded in clinical best practices and research. Collaborate with medical providers to monitor patient progress and adapt treatment plans as necessary, ensuring a unified approach to patient care. Provide crisis intervention and support during mental health emergencies, applying sound judgment to stabilize patients in distress. Educate patients on the relationship between their mental and physical health, empowering them to take active roles in managing their conditions. Maintain precise and confidential documentation in accordance with legal and ethical standards using our integrated electronic health record (EHR) system. Participate actively in team meetings, sharing insights and strategies for enhancing patient care as a member of our interdisciplinary team. Stay informed about current research, techniques, and advancements in mental health and integrated care to continuously enhance your clinical practice. Patient Population / Client Focus You will primarily work with individuals living with complex medical conditions and psychological challenges including, but not limited to, anxiety, depression, chronic pain, and substance use issues. Your role will be vital in understanding how these co-occurring conditions can impact a patient's overall health and effectiveness in managing them. Qualifications A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required. Current licensure as an LCSW, LPCC, or LMFT in the state of California. Expertise in providing mental health care for individuals with co-occurring medical conditions. Demonstrated experience in integrated care and effective collaboration with medical personnel. Strong interpersonal and communication skills with a knack for building productive relationships with patients and team members. Commitment to maintaining professional boundaries and upholding ethical standards in patient confidentiality. Familiarity with electronic health record systems and the ability to navigate various healthcare applications effectively. A collaborative mindset and a proven ability to thrive within a multidisciplinary team setting. Preferred Experience Prior experience in an outpatient setting specifically addressing integrated care. Exposure to crisis intervention techniques and emergency management protocols. A proactive approach to continuing education, holding certifications relevant to mental health practice or integrated care. Compensation and Benefits This position offers a competitive annual salary ranging from USD 75,000 to 104,000, commensurate with experience.
