About This Role

The Care Manager (RN) role involves leading complex care management for members with primarily physical health needs through comprehensive assessments, person-centered care planning, and ongoing monitoring. This position is ideal for RNs looking to support Medicaid members and collaborate with cross-functional teams.

What You\'ll Do

• Evaluate member needs and barriers to accessing appropriate care.
• Develop ongoing care plans and collaborate with providers.
• Identify problems/barriers to care and provide interventions.
• Coordinate between members, families, and care provider teams.
• Provide ongoing follow-up and monitoring of member status.
• Offer resource support for local services based on assessments.
• Facilitate care management and ensure timely access to services.
• Perform outreach to assess member needs and collaborate with resources.
• Collect, document, and maintain member information for compliance.
• Educate members and families on disease processes and care options.

Requirements

Education:

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.

Experience:

• 2 - 4 years of related experience required.

Licenses & Certifications:

• RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
