Work Environment\n

As a fully remote position, this role offers the flexibility of working from home while still being an integral part of our community health clinic team. The work schedule is structured from Monday to Friday, with hours from 8:00 AM to 5:00 PM, with the potential for occasional evening shifts as patient needs dictate. Key Responsibilities\n

• Conduct comprehensive mental health assessments and evaluations, taking into account the interplay between medical and psychological conditions that affect patients' well-being.

• Develop, implement, and monitor integrated treatment plans that address the psychological and medical needs of patients, ensuring a holistic approach to care.

• Facilitate individual and group therapy sessions, employing evidence-based therapeutic modalities tailored to each patient's specific circumstances.

• Collaborate closely with medical providers, including physicians and nurses, to enhance treatment outcomes through regular communication and coordinated care efforts.

• Provide immediate crisis intervention and support during mental health emergencies, ensuring the safety and stability of patients.

• Educate patients on the interrelatedness of physical and mental health, equipping them with tools to better manage their mental health and medical conditions.

• Maintain meticulous and confidential documentation in accordance with legal and ethical standards, utilizing our electronic health record (EHR) system.

Patient Population / Client Focus\n

The patients you will serve typically face co-occurring mental health and medical conditions, including but not limited to chronic pain, substance use disorders, and autoimmune diseases. Your expertise will help bridge the gap between mental and physical health, allowing for improved patient outcomes in a comprehensive, empathetic care setting.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• A current and valid licensure as an LCSW, LCPC, or LMFT in the state of Illinois.

• Proven ability to effectively address the mental health needs of patients with interrelated medical conditions.

• Experience in providing integrated care and working collaboratively with a variety of healthcare professionals.

• Strong interpersonal and communication skills with a commitment to high standards of patient confidentiality and ethical conduct.
