Key Responsibilities\n

• Conduct comprehensive mental health assessments, taking into consideration how various medical conditions impact patients' mental well-being.

• Develop and execute personalized treatment plans that integrate both mental health and medical needs, ensuring coordinated communication across disciplines.

• Facilitate individual and group therapy sessions utilizing evidence-based practices tailored to the specific circumstances and requirements of each patient.

• Provide crisis intervention support, following established protocols to help patients in mental health emergencies.

Patient Population / Client Focus\n

You will primarily work with clients who have co-occurring disorders, including but not limited to chronic pain, autoimmune diseases, and substance use issues. This role demands sensitivity and awareness of the complexities faced by patients who experience both medical and mental health challenges, allowing you to create individualized interventions that lead to improved overall wellness.

Qualifications\n

• Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current licensure as an LCSW, LCPC, or LMFT within the state of Illinois.

• Strong understanding and experience in working with individuals facing co-occurring medical and psychiatric conditions.

• Excellent communication skills, with a proven capacity to maintain professional boundaries and adhere to ethical practices.

• Proficiency in utilizing electronic health record (EHR) systems and engaging with various digital tools for remote counseling.

• A collaborative approach to care, emphasizing teamwork within an interdisciplinary setting to achieve the best patient outcomes.

• At least two years of experience in a clinical setting, preferably in an integrated or multidisciplinary care environment.

Compensation and Benefits\n

This position offers a competitive salary ranging from $75,000 to $100,000 annually, reflecting your experience and expertise.
