Key Responsibilities\n

• Conduct detailed psychosocial assessments through interviews and observations, in addition to reviewing medical records, to gain a comprehensive understanding of factors affecting students’ emotional, social, and environmental contexts.

• Provide empathetic counseling and support to both students and their families, helping them navigate adjustment issues and overcome emotional hurdles associated with rehabilitation.

• Collaborate closely with multidisciplinary rehabilitation teams to formulate individualized treatment plans that specifically address the psychosocial needs of each student.

• Facilitate access to community resources, ensuring students can connect with support groups, vocational rehabilitation services, or housing assistance as necessary.

• Engage in discharge planning, providing guidance and support to create seamless transitions for students returning home or moving to alternative care settings.

• Educate families and students about available community resources and post-rehabilitation services to foster greater independence and well-being.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program.

• Current state licensure as a Licensed Clinical Social Worker (LCSW) in Colorado.
