Key Responsibilities

Your responsibilities in this role will include:

• Conducting comprehensive evaluations of patients to assess their physical condition, medical history, and rehabilitation needs. Using your expertise, you will document findings clearly in patient files.
• Developing and explaining individualized treatment plans to patients and their families, ensuring their involvement in the goal-setting process to enhance engagement and outcome effectiveness.
• Implementing a variety of therapeutic exercise protocols and therapeutic modalities tailored to the unique needs of each patient, leveraging your skills to address pain management and mobility issues.
• Monitoring patient progress and modifying treatment plans as necessary, in collaboration with other healthcare providers, to adapt to any changes in patient condition.
• Engaging in detailed discharge planning to facilitate smooth transitions to home or secondary care, and documenting all patient interactions, evaluations, and treatment provided in compliance with healthcare regulations.
