Key Responsibilities\n

• Conduct thorough assessments and diagnostic evaluations to address the unique needs of each patient.

• Maintain regular communication with supervisors, interdisciplinary team members, family, and guardians to facilitate coordinated care and share updates on client progress.

• Develop and manage a tentative schedule of appointments, including virtual sessions and home visits, ensuring clients receive ongoing support.
\n
• Coordinate and participate in home visits to document observations, conduct chart reviews, and engage in staff interviews for comprehensive case management.
\n
• Actively participate in Interdisciplinary Team Meetings and contribute insights during various discussions on client care.
\n
• Complete detailed clinical documentation, including monthly and quarterly progress notes that track patient development and inform future care strategies.
\n
• Lead training sessions to ensure staff members are proficient in implementing program plans and maintaining updated documentation practices.
\n
• Conduct follow-up interviews and home visits to ensure clients are receiving consistent support tailored to their evolving needs.

• Maintain organized records to ensure compliance with legal and ethical standards, making necessary information available to authorized personnel.

• Serve as a liaison with community partners and mental health organizations to foster collaboration in achieving developmental goals.

• Contribute to the development of training materials aimed at enhancing service delivery and client outcomes.

Patient Population / Client Focus\n

As a Licensed Professional Counselor, you will work with a broad array of clients, including children, adolescents, and adults facing various mental health challenges. Your role will involve engaging with individuals dealing with issues such as anxiety, depression, trauma, substance misuse, and relational conflicts. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
