This role will be conducted remotely, allowing you to balance your professional responsibilities with flexibility. You'll be an integral part of a Community Health Clinic that focuses on providing vital services to children and families experiencing developmental delays or disabilities. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to identify developmental needs and tailor individualized intervention strategies.
\n
• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), collaborating with families to establish achievable goals.
\n
• Monitor and document outcomes as part of a multidisciplinary team, facilitating adjustments to intervention plans based on progress.
\n
• Provide education and support to families, equipping them with tools and strategies to foster their child's development in a natural learning environment.
\n
• Utilize coaching techniques to empower families, emphasizing evidence-based practices that are adaptable to varied learning styles.
\n
• Ensure compliance with all Federal and State regulations relevant to social work practices, prioritizing the health and safety of clients and staff members.
\n
• Develop productive relationships with children, families, and team members to nurture a supportive and effective learning atmosphere.

• Recommend the frequency of services based on ongoing assessments of children’s development and family priorities.

Patient Population / Client Focus\n

Your primary focus will be on children aged birth to three who are experiencing developmental challenges. By providing early intervention services, you will help these young individuals develop essential physical, cognitive, social/emotional, and communication skills during their critical developmental window. Your supportive role will also extend to families, ensuring they have the knowledge and resources necessary to aid their children's growth.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work (MSW/DSW) is mandatory.

• Current licensure as a Clinical Social Worker (LCSW) in compliance with state regulations.
\n
• A valid driver’s license is required for potential home visits or community engagements.

• Proficiency in adapting interventions to meet diverse learning styles is essential.

• One year of direct experience in providing early intervention services to children is required.
