Key Responsibilities\n

• Conduct thorough assessments of patients' psychosocial needs, establishing suitable treatment plans that encompass case management, short-term counseling, and crisis intervention strategies.

• Provide assistance to patients in navigating the completion of forms related to entitlements, supplemental insurance, and other benefits aimed at reducing reliance on emergency support services.
\n
• Facilitate access to prescription cost assistance by connecting patients with community resources and pharmaceutical programs designed to lessen financial burdens related to medication.
\n
• Act as a patient advocate when engaging with community agencies to ensure clients receive the necessary resources and support.
\n
• Maintain comprehensive and updated patient records, preparing essential reports and documentation in adherence to confidentiality and regulatory guidelines.
\n
• Coordinate transportation assistance for patients as needed to ensure they can attend their therapy sessions and appointments.
\n
• Participate actively in clinical and team meetings, contributing insights during case conferences to enhance collaborative care efforts.

Patient Population / Client Focus\n

In this role, you will work with a diverse client population, ranging from children and adolescents to adults facing various mental health challenges, including anxiety, depression, trauma, and relationship issues. Your expertise will guide individuals and families through their mental health journeys while providing them with the tools and support they need to foster resilience and achieve personal goals.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.
\n
• Possession of one of the following independent state licenses: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• At least two (2) years of professional experience in a mental health counseling capacity, demonstrating ability to work with a varied clientele.

Preferred Experience\n

Some additional experience with telehealth platforms, familiarity with digital documentation systems, and knowledge of community resources for mental health support are preferred but not mandatory.

Compensation and Benefits\n
