Key Responsibilities\n

As a Licensed Professional Counselor, your responsibilities will include:

• Conducting thorough and ongoing assessments to evaluate the mental health needs of patients.

• Regularly communicating with supervising staff, direct care team members, family members, and guardians to discuss individual progress, ensuring continuity of care.

• Providing supervisors with a tentative schedule of appointments, home visits, and necessary observations to facilitate effective treatment planning.

• Coordinating and conducting home visits for in-home observations, comprehensive chart reviews, and staff interviews as needed.

• Ensuring that the implementation of treatment plans is carried out effectively and consistently.
\n
• Actively participating in Interdisciplinary Team Meetings to collaborate on treatment strategies and share insights.
\n
• Writing and completing monthly and quarterly progress notes to track patient improvements and outcomes.
\n
• Organizing and delivering training sessions to certify staff members in program plan implementations and documentation protocols.
\n
• Providing follow-up interviews and support through home visits, ensuring that individual needs are met continuously.

• Maintaining accurate and confidential records for easy access by authorized personnel and stakeholders.

• Acting as a representative and liaison between the organization and the mental health community to promote development goals.

• Assisting in the creation of training materials aimed at improving programs for clients.

Patient Population / Client Focus\n

You will have the opportunity to work with a diverse array of client demographics, including both adults and youth, focusing on a range of mental health challenges such as anxiety, depression, family dynamics, and trauma recovery. Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Colorado.

• A minimum of 2 years of counseling experience in a clinical context is essential.
