Work Environment

This position is onsite, focusing on home care service delivery. Our therapists work closely within the community, collaborating with healthcare professionals and visiting patients' homes to implement tailored rehabilitation programs. Key Responsibilities

• Conduct thorough evaluations of patients, assessing their physical limitations and designing individualized treatment plans to enhance mobility and function.
• Communicate effectively with patients and their families about rehabilitation goals, explaining findings in accessible terms, and integrating their feedback into treatment objectives.
• Implement and monitor treatment protocols, adjusting strategies based on ongoing assessments and the unique responses of each patient.
• Collaborate with a multidisciplinary team, including physicians and nursing staff, to ensure holistic patient management and timely adjustments to care plans as conditions evolve.
• Document all patient interactions meticulously, ensuring clarity and compliance with healthcare regulations, including initial evaluations, progress notes, and discharge plans.
• Maintain productivity standards by effectively managing your schedule, being punctual for all appointments, and participating in departmental meetings.
• Engage in community outreach and marketing initiatives to promote our services and enhance community accessibility to quality care.

Patient Population / Client Focus

The primary focus of our Physical Therapist role involves working with a diverse patient demographic, including elderly patients recovering from surgery, individuals with chronic pain conditions, and those undergoing rehabilitation after injuries. Our therapists are integral in helping patients regain independence and improve their functional abilities within their home environments, addressing both physical and emotional needs.

Qualifications

• Doctorate degree in Physical Therapy (DPT) required.
• Current and valid state license to practice as a Physical Therapist in Virginia.
• Certification in Basic Life Support (BLS) by the American Heart Association is mandatory.
• Excellent communication and interpersonal skills to engage effectively with patients and families.

Preferred Experience

• Minimum of two years of experience in a clinical setting is preferred, although exceptional new graduates with internships or clinical rotations in home health settings will also be considered.

Compensation and Benefits
