Key Responsibilities\n

• Conduct thorough, culturally sensitive bio-psychosocial assessments on patients to identify mental health, substance abuse, and medical needs within a primary care context.

• Provide a range of behavioral health services, including individual, couple, and family counseling. This encompasses initial intake assessments, diagnostic evaluations, treatment planning, crisis intervention, and referrals for community-based treatment options.

• Develop and maintain comprehensive progress notes and clinical documentation electronically, ensuring that patient records are accurate and up-to-date.

• Collaborate effectively with the Quality Improvement teams to develop and implement data collection processes, aiming to improve service delivery and patient outcomes.

• Serve as a liaison to connect patients with social work support, community resources, and medication assistance programs as necessary.

Patient Population / Client Focus\n

The Licensed Clinical Social Worker will work primarily with individuals experiencing substance use disorders and mental health challenges, including co-occurring disorders. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
