Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

An immediate opportunity is available for a Home Infusion Clinical Pharmacist with expertise in sterile compounding, IV specialty medications, and patient-centered care. This direct hire role is designed for pharmacists with at least two years of infusion pharmacy experience who thrive in a clinical and collaborative environment.

Schedule and Location:

• Monday through Friday, 8:30 AM to 5:00 PM
• Participation in on-call rotation
• Located in Dublin, OH

Key Responsibilities:

• Prepare specialty pharmaceuticals and intravenous solutions in a sterile environment, adhering to all state and federal regulations.
• Monitor clinical parameters including lab values and pharmacokinetic dosing (e.g., vancomycin), adjusting therapies such as Total Parenteral Nutrition (TPN) accordingly.
• Provide detailed patient counseling and education on specialty medication regimens and infusion pump use, ensuring safe and effective home therapy.
• Collaborate with prescribing physicians, home health nurses, and multidisciplinary teams to coordinate care and avoid hospital readmissions.
• Maintain full compliance with regulatory requirements including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA standards.
• Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for timely home delivery.
• Utilize the CareTend system to support pharmacy operations and patient management.

Qualifications:

• Active and current pharmacist license in Ohio, in good standing.
• Minimum two years’ experience in infusion specialty pharmacy.
• IVIg therapy experience is a plus.
• Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
