Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A full-time clinical pharmacist role is available to support home infusion therapy with a direct hire opportunity starting ASAP. This position requires a skillful pharmacist to prepare, dispense, and closely monitor complex intravenous (IV) and subcutaneous medications for patients receiving home infusion therapy.

Schedule:

• Monday to Friday, 8:30 AM to 5:00 PM
• Participate in on-call rotation

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions aseptically in compliance with state and federal regulations.
• Assess laboratory values and calculate pharmacokinetic dosing, including drugs like vancomycin, and manage Total Parenteral Nutrition (TPN).
• Provide comprehensive patient counseling and education on specialty medications, infusion pump use, sterile technique, and catheter care.
• Collaborate with physicians, nurses, and pharmacy teams to coordinate care and prevent hospital readmissions.
• Ensure regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
• Manage pharmacy inventory, including refrigerated items, and prepare medications and supplies for secure and timely home delivery.

Qualifications and Desired Experience:

• Minimum of 2 years of infusion specialty pharmacy experience
• IVIg experience is a strong plus
• Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
• Current and good standing Ohio pharmacist license
• Experience with the CareTend system preferred
