Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, identifying emotional, mental, and social challenges relating to their substance use history.

• Collaborate with clients to develop tailored care plans that reflect their personal goals, preferences, and the strategies needed to facilitate their recovery.

• Provide one-on-one counseling and emotional support to individuals and families navigating the complexities of detoxification and recovery.

• Serve as an advocate for clients, ensuring their rights are protected while facilitating their access to relevant community resources and support services.

• Participate in interdisciplinary team meetings to coordinate comprehensive treatment plans and improve overall client care outcomes.

• Monitor clients’ progress through regular virtual check-ins, adjusting care plans as necessary based on individual needs and recovery trajectories.

• Facilitate family meetings to address any concerns and to educate relatives about the detox process and available community resources.

• Maintain detailed documentation of assessments, care plans, and client interactions within the EHR system, ensuring compliance with legal and ethical standards.

• Stay updated on best practices and the latest policies related to social work, substance use treatment, and eldercare to provide informed care.

Patient Population / Client Focus\n

Your primary focus will be on seniors facing substance use challenges, often compounded by the complexities of aging. This role requires empathy and a deep understanding of age-related issues, including mental health concerns, underlying medical conditions, and the social dynamics surrounding older adults.

Qualifications\n

• Master's Degree in Social Work from an accredited institution.

• Current and valid LCSW license from the state of New Jersey.

• A minimum of two years of professional experience as a social worker, particularly in settings focused on addiction, mental health, or geriatric care.

Compensation and Benefits\n

We offer a competitive salary range between $60,000 and $100,000 per year, commensurate with experience.
