Work Environment\n

The successful candidate will work remotely, engaging with families and teams through virtual platforms. This setup allows for flexibility while ensuring that our Licensed Clinical Social Workers (LCSWs) can maintain continual support for those we serve. As part of a Hospice Center, you will be a crucial player in a team-oriented environment, including collaborating with healthcare professionals and family members to optimize care delivery.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children and families to identify needs and develop strategic plans tailored to individual situations.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), monitoring progress and adjusting methodologies as necessary.

• Administer evidence-based interventions and provide coaching to families that foster child development in natural environments.
\n
• Collaborate with multidisciplinary teams to ensure a holistic approach to care that meets regulatory standards and adheres to best practices.

• Develop trusting relationships with families, ensuring they receive not only clinical support but practical resources and education related to enhancing their child's development.

• Engage in continuous professional development to remain current with state and federal regulations impacting social work and child welfare.

Patient Population / Client Focus\n

This position focuses on infants and toddlers, particularly those with disabilities or developmental delays. Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.
