Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments to understand the complexities of each patient's emotional and social needs.

• Develop and implement individualized treatment plans utilizing evidence-based practices and therapeutic modalities tailored to each patient.

• Provide ongoing therapy sessions, offering both individual and group therapies designed to enhance coping strategies and emotional well-being.

• Facilitate multidisciplinary team meetings to ensure coordinated care, sharing insights regarding patients' psychosocial status and recommending appropriate interventions.

• Document all case interactions and treatment progress meticulously in patients' medical records, adhering to departmental and legal standards.

• Educate families about the social work process and available resources, helping them understand and embrace treatment recommendations effectively.

• Advocate for patients within the community, identifying and coordinating necessary support services to enhance their quality of life and social integration.

Patient Population / Client Focus\n

In this role, you will primarily work with patients facing serious illnesses, particularly those in hospice care. You will support caregivers and families in dealing with emotional, social, and spiritual distress, fostering an environment where open communication about grief, loss, and acceptance is encouraged. Qualifications\n

• A Master's Degree in Social Work from an accredited program.

Compensation and Benefits\n

The salary range for this position is between USD 85,000 and 100,000 annually, commensurate with experience and qualifications.
