Key Responsibilities\n

• Conduct comprehensive virtual psychosocial assessments to evaluate clients’ strengths, challenges, and needs, ensuring a thorough understanding of their situation.

• Develop and implement individualized treatment plans that include evidence-based therapeutic interventions tailored to clients’ specific circumstances.

• Facilitate individual and group therapy sessions using various modalities to support clients in coping with life-limiting illnesses and emotional distress.

• Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare providers, to coordinate integrated care aimed at promoting clients' overall wellness.

• Advocate for clients, ensuring they have access to necessary community resources and support services to enhance their quality of life.
\n
• Document client interactions, treatment progress, and outcomes in electronic health records, maintaining accurate and timely records to support continuity of care.

• Participate in team meetings, case reviews, and ongoing professional development to continuously enhance your clinical skills and knowledge.

Patient Population / Client Focus\n

The individuals you will support primarily consist of patients in hospice care, their families, and caregivers navigating the emotional and psychological challenges associated with end-of-life issues. Understanding and addressing the unique needs of grieving families, terminal clients, and supporting them through complex emotional landscapes is central to this role.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.

• A minimum of two years of post-licensure experience providing clinical social work services, with a focus on mental health and hospice care.

• Strong proficiency in conducting diagnostic assessments and developing culturally competent treatment plans.

• Excellent interpersonal and communication skills, with a demonstrated ability to engage effectively with a diverse range of clients.
