Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, including reviewing medical history, performing specific assessments, and documenting findings accurately.
• Communicate evaluation results and treatment plans clearly to patients and their families, ensuring they are actively involved in the goal-setting process.
• Implement individualized treatment protocols informed by evaluation outcomes and continually assess the patients' progress, making modifications as necessary.
• Consult with other healthcare providers as needed, identifying any changes in a patient's condition that may require adjustments in the treatment plan.
• Document all patient interactions and care activities meticulously, including initial evaluations, progress notes, and discharge summaries, to provide a clear picture of each patient's treatment journey.
• Demonstrate effective time management by maintaining an accurate patient schedule, meeting productivity requirements, and attending all necessary meetings punctually.
• Contribute actively to departmental goals and comply with all regulatory and accreditation standards to ensure a safe and effective treatment environment.
• Pursue continuing education opportunities and engage in professional development activities to enhance personal competencies and organizational effectiveness.
• Improve patient access and experience by demonstrating flexibility and adaptability in care delivery settings and actively participating in community outreach initiatives.

Patient Population / Client Focus

You will primarily work with individuals recovering from surgeries, injuries, and chronic conditions that affect mobility and function. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
