Key Responsibilities\n

• Conduct ongoing assessments of clients to identify needs and create tailored treatment plans.

• Regularly communicate progress and updates among supervisors, direct care team members, family members, and guardians to ensure a unified approach to clinical care.

• Develop and provide a tentative schedule of appointments and home visits to effectively monitor client's progress.

• Coordinate home visits, including thorough chart reviews and staff interviews, to gain insights into client functioning in their natural environment.

• Oversee the implementation of individualized program plans, ensuring that all actions support the therapeutic process.

• Participate actively in Interdisciplinary Team Meetings, sharing insights that can enhance client care.
\n
• Document treatment outcomes through meticulous monthly and quarterly progress notes that reflect the client's journey.

• Organize staff training to ensure adherence to program guidelines and compliance with documentation standards.
\n
• Conduct follow-up interviews and observations to continue supporting clients' evolving needs.

• Maintain comprehensive records, ensuring that all relevant information is accessible to authorized parties.

• Serve as a liaison between clients, families, and community mental health resources, facilitating connections to support services.

• Contribute to the development of training materials aimed at enhancing program effectiveness.

Qualifications\n
