Key Responsibilities\n

As a Licensed Mental Health Counselor at Rocky Meadow Counseling, your daily responsibilities will include, but are not limited to:

• Conducting comprehensive psychosocial assessments to accurately diagnose and treat a range of mental health disorders, including anxiety, depression, and trauma-related issues.

• Developing individualized treatment plans that are regularly updated to reflect patient progress and changing needs.

• Collaborating with a multidisciplinary team to ensure continuity of care and effective discharge planning for patients transitioning from outpatient services.

• Providing referrals to community resources, treatment programs, and support networks to enhance patient well-being.
\n
• Organizing and prioritizing your daily caseload while assessing the needs of new, ongoing, and discharging patients.

• Facilitating therapeutic sessions with both individuals and families, offering support, crisis intervention, and guidance tailored to their unique situations.

• Actively participating in care coordination amongst healthcare providers, insurers, and various stakeholders to resolve any barriers or challenges impacting patient care.

• Acting as a patient advocate to ensure effective communication and collaboration among all members of the care team.

• Staying compliant with all federal, state, and local regulations, along with maintaining adherence to clinical standards of practice.

Patient Population / Client Focus\n

The counselor will work primarily with adults and adolescents experiencing a variety of mental health challenges. Qualifications\n

Compensation and Benefits\n

The position offers a competitive annual salary range of USD 90,000 – 100,000, commensurate with experience and qualifications. Additionally, we provide:
