Work Environment

This role is primarily onsite in a home care setting, enabling you to work closely with patients in the comfort of their own homes. Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical abilities and limitations, including range of motion, strength, and functional independence.
• Develop, implement, and modify individualized treatment plans based on the evaluation findings and in collaboration with other healthcare providers as needed.
• Provide continuous assessment of patients' progress and adapt treatment strategies to ensure maximum effectiveness and patient satisfaction.
• Educate patients and their families on treatment plans, exercises, and self-management strategies to promote active participation in their recovery process.
• Document all patient interactions and treatment procedures accurately and in compliance with applicable laws, regulations, and professional standards.
• Collaborate with interdisciplinary team members, including physicians and nursing staff, to coordinate patient care and enhance outcomes.
• Participate in community outreach initiatives to raise awareness of physical therapy services and promote overall wellness in the community.

Compensation and Benefits
