Work Environment

This full-time position is primarily onsite in the patient's homes throughout Orlando, allowing for a personalized approach to rehabilitation. Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories, performing specific assessments, and documenting evaluation results effectively.
• Engage with patients and their families to explain evaluation findings and collaboratively develop treatment plans that align with personal goals and needs.
• Implement evidence-based treatment protocols tailored to each patient's progress and condition, ensuring adherence to best practices within the evolving healthcare landscape.
• Monitor changes in patient conditions, adjusting care plans accordingly, and collaborating with other healthcare professionals when needed for optimal outcomes.
• Manage discharge planning and aftercare, meticulously documenting all patient care activities, including evaluations, daily treatment notes, and progress summaries.

Patient Population / Client Focus

Qualifications
