Key Responsibilities

• Conduct thorough psychosocial assessments of patients to identify emotional, social, and financial needs, collaborating closely with interdisciplinary medical teams to create integrated care plans.
• Provide both individual and group counseling sessions to help patients and their families navigate adjustment issues, grief, and coping strategies related to their health challenges.
• Coordinate with community organizations to develop post-discharge services, ensuring a seamless transition and continuity of care for patients.
• Offer emotional support and education to patients and families about treatment options, healthcare resources, and coping mechanisms.
• Participate actively in hospital committees, bringing expertise on social work matters to improve overall patient care and operational effectiveness.
• Provide crisis intervention and emotional support, implementing appropriate strategies to assist patients in distress.
• Develop and implement comprehensive discharge plans, connecting patients with necessary community resources.
• Maintain accurate documentation in compliance with healthcare regulations, ensuring that patient records reflect timely and relevant information.

Patient Population / Client Focus

You will primarily work with patients dealing with terminal illnesses and their families, ensuring that emotional and psychological support is integral to their care. Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits

The salary for this role ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
