Work Environment\n

As a remote working position, this role allows you to conduct client visits from the comfort of your home while adhering to the flexibility required for providing optimal care. Your schedule will include a full-time commitment, from Monday to Friday, 8:00 AM to 4:30 PM, with occasional weekends or evenings based on client needs. The remote setting provides opportunities for both on-site client assessments and home visits, fostering a close-knit virtual team culture that prioritizes collaboration and communication.

Key Responsibilities\n

• Conduct in-depth assessments of seniors’ psychosocial needs, ensuring a comprehensive understanding of their emotional, mental, and social health.

• Collaboratively develop tailored care plans with seniors and their families, aligning with their individual goals and preferences to support their journey.

• Provide ongoing counseling and emotional support, assisting clients in navigating life transitions, bereavement, and other significant family dynamics.

• Advocate for clients' rights and ensure access to local community resources, enhancing their overall wellbeing and quality of life.

• Work closely with interdisciplinary teams, including healthcare providers, nurses, and caregivers, coordinating efforts to optimize patient care.

• Facilitate effective communication among family members to resolve conflicts and ensure a unified approach to client care.

Patient Population / Client Focus\n

This position primarily focuses on serving older adults facing chronic illnesses, life-limiting conditions, and transitions related to hospice care. You will work directly with seniors and their families, advocating for their needs while providing the emotional and social support vital for navigating the complexities of aging and health challenges.

Qualifications\n

• Minimum of a Master’s Degree in Social Work from an accredited institution.

• Valid LCSW licensure in the state of Texas.

• A minimum of two years’ experience in social work, with preference for those who have worked in hospice, geriatric, or community-based settings.

Compensation and Benefits\n
