Key Responsibilities\n

• Provide direct counseling support to patients and families experiencing grief, anxiety, and emotional distress related to hospice care. Tailor interventions based on individual circumstances to ensure a compassionate and supportive experience.

• Conduct thorough psychosocial assessments, including gathering comprehensive case histories and employing effective interview techniques to understand the diverse needs of clients.

• Provide case management services, ensuring that patients receive necessary resources and support tailored to their unique situations and challenges.

• Utilize evidence-based practices to monitor client progress and adjust treatment plans as necessary, while actively tracking outcomes to enhance service delivery.

• Proactively identify and assess risks, including the de-escalation of potential crisis situations, and appropriately escalate concerns to the clinical team and emergency services when required.

• Stay apprised of current safeguarding policies and procedures, integrating them into daily practices while advocating for clients' rights and protections.

• Contribute insights to leadership regarding emerging trends and challenges within the service, recommending changes to improve patient outcomes.

Patient Population / Client Focus\n

This position involves working with a sensitive patient population comprising individuals of all ages facing terminal illnesses, as well as their families. You will address a variety of emotional and mental health challenges related to grief, loss, and coping with impending transitions, requiring empathy, compassion, and expertise.

Qualifications\n

• Master's Degree in Social Work from an accredited graduate program.

• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

• A minimum of two years of professional experience in a behavioral health or social work setting, preferably with a focus on end-of-life care.

Compensation and Benefits\n
