Key Responsibilities\n

• Initiate discharge planning on the same day of admission for all patients, ensuring that every client has a clear path to recovery.

• Conduct thorough discharge sessions with clients to confirm their understanding of coping strategies, transition plans, and available resources.
\n
• Coordinate and confirm discharge appointments for clients and their family members, ensuring readiness for transition.
\n
• Engage proactively with referral sources and outpatient providers to facilitate continuity of care throughout different phases of treatment.

• Assist in patient transfers to higher levels of care when necessary, advocating for client needs during the process.
\n
• Participate actively in clinical outreach and program promotional activities to enhance community awareness of available mental health resources.
\n
• Effectively manage crises during sessions, providing a safe environment for clients and intervening as required.
\n
• Support milieu staff in their daily interactions with clients and families, promoting a therapeutic community.
\n
• Collaborate with the treatment team to uphold the highest standards of clinical quality and patient care.

• Complete necessary documentation, including initial assessments, treatment plans, and case notes, thoroughly and promptly.
\n
• Assess patients for potential suicidality and evaluate risks to self or others, implementing appropriate interventions when necessary.

• Provide supplementary clinical support to team members when needed, demonstrating flexibility and teamwork.
\n
• Maintain clear communication with the Utilization Review Department regarding authorizations and payment processes.
