Role Description The Patient Care Coordinator is a full-time, on-site role based in Melville, NY. This position is responsible for greeting patients, managing check-in and check-out processes, and ensuring that all patient information is accurately collected and updated. Day-to-day tasks include scheduling and confirming appointments, answering phone calls, responding to patient inquiries, and coordinating follow-up visits or referrals as directed by clinical staff. The coordinator helps support providers by organizing patient flow, maintaining thorough documentation, and reinforcing pre- and post-visit instructions. This role also involves collaborating with the broader care team to promote a smooth, respectful, and efficient patient experience.
