Key Responsibilities\n

• Conduct thorough psychosocial assessments and evaluations of seniors and their families to identify individual needs and develop tailored care plans.

• Collaborate with healthcare professionals and family members to design and implement Individualized Family Service Plans (IFSPs) that promote the client’s well-being.

• Engage in regular follow-ups to monitor client outcomes and measure the effectiveness of interventions, making necessary adjustments to care plans.

• Provide emotional support and counseling to clients and families in crisis situations, ensuring a compassionate approach to difficult discussions.

• Maintain compliance with all Federal and State regulations, as well as agency policies, contributing to a high standard of ethical practice.

• Develop strong rapport with clients and their families, fostering a supportive environment that encourages open communication and trust.

• Utilize case management software and electronic health records (EHR) systems to document assessments, interventions, and progress.
