Key Responsibilities\n

• Conduct thorough psychosocial assessments, gathering crucial information regarding each client's mental health status, needs, and social environment.

• Design and implement personalized therapy plans that utilize evidence-based treatment modalities, including cognitive-behavioral therapy and solution-focused therapy.

• Facilitate individual, family, and group therapy sessions, crafting a safe space for healing and open dialogue about end-of-life issues.

• Work collaboratively with a multidisciplinary team, including physicians and nursing staff, to ensure an integrated approach to patient care.

• Provide crisis intervention as needed, assessing risks and emergency planning to secure clients' wellbeing during critical moments.

• Maintain accurate documentation compliant with ethical standards, safeguarding client confidentiality and information integrity.

• Facilitate group therapy, psychoeducation, and support groups to promote community and shared experiences among clients.

Patient Population / Client Focus\n

In this role, you will support a diverse population, helping patients who are in hospice care manage emotional and psychological challenges related to their diagnosis. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Connecticut.

Compensation and Benefits\n
