Key Responsibilities\n

• Conduct comprehensive psychosocial assessments, collecting detailed information about clients' mental health concerns, strengths, and resources.

• Provide individualized therapy sessions, as well as family and group therapy, using evidence-based modalities such as Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, and Trauma-Focused Therapy.

• Collaborate closely with psychiatrists, psychologists, and other interdisciplinary team members to create and implement comprehensive treatment plans that promote client-centered care.

• Facilitate crisis intervention strategies, conduct risk assessments, and develop safety plans as required.
\n
• Maintain both timely and accurate documentation of client interactions and treatment outcomes, adhering to industry best practices and legal standards.

• Lead group therapy sessions, psychoeducation classes, and support groups to provide community and connection among clients.

Patient Population / Client Focus\n

Our client population includes individuals of diverse backgrounds experiencing various mental health challenges, particularly those facing end-of-life issues within a hospice context. As a Licensed Clinical Social Worker, you will play a pivotal role in addressing the emotional and psychological needs of clients and their families, providing them with essential coping strategies and emotional support during some of life’s most critical moments.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW)
