Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients' mental health concerns, strengths, and available resources, ensuring a client-centered approach.

• Deliver individual, family, and group therapy utilizing evidence-based treatment modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.

• Develop and implement tailored treatment plans that incorporate measurable goals and evidence-based interventions, adjusting as necessary based on patient progress.

• Work collaboratively with interdisciplinary teams, including psychiatrists and other healthcare providers, to coordinate comprehensive care that addresses both physical and emotional needs.

• Facilitate group therapy sessions, psychoeducational workshops, and support groups, fostering community and shared healing experiences among patients.

• Conduct risk assessments and crisis interventions as needed, ensuring patient safety and well-being at all times.

• Maintain accurate and timely documentation in compliance with ethical and regulatory standards, ensuring confidentiality and professionalism in all client interactions.

Patient Population / Client Focus\n

Your primary focus will be on providing mental health support to clients facing life-limiting illnesses and their families within a hospice care setting. This includes individuals who are experiencing complex and emotional responses to terminal diagnoses, and those who may be dealing with feelings of sadness, anxiety, or grief as they navigate end-of-life decisions. Understanding the sensitive nature of this population is essential to effectively delivering therapeutic care and support.

Qualifications\n

• A Master's degree in Social Work from a program accredited by the Council on Social Work Education.

• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of three years of experience in a mental health or outpatient setting, with direct experience in hospice or palliative care preferred.

Compensation and Benefits\n
