Additional Unit Specific Information: End of life care assessment and care planning requiring strong comprehensive assessment skills. Symptom assessment and management with high level of critical thinking skills. Comfort initiating and participating in goals of care discussions and planning with patients, family, and team. Strong communication skills and ability to teach patients and caregivers aspects of end-of-life care. Generally, a traveler case manager is assigned a geographic area, not full territory. When they cover a weekend rotation-they would be expected to make visits throughout our service area- Chittenden & Grand Isle Countries. We have had travelers that float.
