Key Responsibilities\n

• Engage in regular communication with physicians and other healthcare professionals to report on patient progress, identify needs, and address concerns promptly.

• Consult and coordinate treatment plans with families and support systems, ensuring a comprehensive approach to client care and effective discharge planning.

• Maintain detailed and timely clinical documentation, including progress notes, in compliance with departmental policies to ensure accurate tracking of patient care.

• Collaborate with the healthcare team to develop thorough treatment plans based on psychosocial assessments and recognized therapeutic interventions.

• Assist clients in discharge planning by coordinating care and liaising with various regional healthcare providers to facilitate access to necessary services and resources.

Qualifications\n

• Education: A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or a relevant field is required.

• Licensure: Active state licensure as an LCSW, LMFT, LPC, or equivalent credential is necessary to ensure compliance with regulatory standards.

• Experience: A minimum of 3 years of experience in an outpatient mental health setting is required, demonstrating the ability to handle a variety of clinical cases effectively.

• Technical Skills: Proficiency in electronic medical record systems, word processing, email communication, and spreadsheet management is essential to facilitate accurate documentation and communication.

• Organization: Strong independent work ethic, self-direction, and organizational skills are crucial for managing a caseload effectively in a remote environment.

Preferred Experience\n

Prior experience in an outpatient clinic setting, along with knowledge of evidence-based therapeutic modalities such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT), is preferred. Familiarity with substance abuse treatment protocols will be an added advantage.

Compensation and Benefits\n
