Key Responsibilities\n

• Conduct in-depth psychosocial assessments to gather critical information about clients' mental health status, emotional strengths, and challenges.

• Provide individual, family, and group therapy sessions, utilizing evidence-based modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.

• Develop, implement, and monitor personalized treatment plans that include measurable intervention goals to facilitate client healing processes.

• Work collaboratively with psychiatrists, psychologists, and the treatment team to coordinate comprehensive care that aligns with clients' therapeutic needs.

• Conduct crisis intervention, risk assessments, and safety planning, especially pertinent in an end-of-life context where emotional support is essential.

• Facilitate therapeutic group sessions and psychoeducational classes, providing support groups tailored to various client needs and backgrounds.

• Ensure accurate and timely documentation of client interactions and treatment progress in compliance with applicable ethical and legal standards.

This role primarily focuses on clients dealing with the emotional and psychological challenges of hospice care. Qualifications\n

• A Master's degree in Social Work from an accredited institution.
