Our Hospice Center environment requires sensitivity and compassion, as you will be dealing with families in challenging situations. You'll have the tools and resources necessary to maintain communication with families and teams to ensure seamless delivery of service.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the needs of children in hospice care and their families.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), making recommendations based on best practices and family input.

• Monitor and document outcomes to ensure the efficacy of interventions, collaborating closely with multidisciplinary team members.
\n
• Provide ongoing emotional and practical support to families, equipping them with strategies and tools to navigate their caregiving challenges effectively.

• Adhere to all Federal and State regulations governing social work practices, ensuring compliance and safeguarding welfare.

• Establish meaningful rapport with children and their families to encourage a positive and empowering caregiving environment.
\n
• Leverage evidence-based practices and natural learning environment strategies to promote the developmental growth of children.
\n
• Utilize the coaching model to facilitate family engagement in the care process while delivering tailored guidance based on individual needs.

• Ensure all interactions comply with health and safety standards, contributing to a secure environment for both staff and clients.

Patient Population / Client Focus\n

Your primary focus will be on infants and toddlers who are receiving hospice care due to terminal illnesses or severe disabilities. This sensitive work involves providing support to their families during some of the most challenging moments of their lives. By utilizing evidence-based practices and individualized service plans, you will ensure that every child receives the appropriate care and resources necessary for their development.

Qualifications\n

• A Master's Degree or Doctorate in Social Work is required.

• Current licensure as an LCSW by the Department of Public Health.

• A valid driver’s license to enable travel as needed for home visits.
\n

Preferred Experience\n

Compensation and Benefits\n
