This position operates in a remote work setting, allowing the LCSW the flexibility to manage their schedule while providing critical emotional support to patients. Key Responsibilities\n

• Provide individualized psychotherapy and evidence-based mental health interventions to patients coping with a variety of mental health conditions, such as anxiety, depression, and PTSD.

• Conduct comprehensive assessments to evaluate each patient’s mental health status and social functioning, using standardized tools and methods.

• Design, implement, and regularly update customized treatment plans based on patient assessments, family input, and evolving health conditions.

• Collaborate with an interdisciplinary team of physicians, nurses, and other healthcare practitioners to promote a cohesive and coordinated treatment strategy.

• Utilize telehealth platforms to deliver therapy sessions, ensuring patients can access support from the comfort of their homes.

• Monitor and document patient progress meticulously in electronic health records, adjusting treatment plans as necessary to reflect changes in patient needs or responses to therapy.

• Educate patients and their families by providing information about mental health conditions and available community resources for ongoing support.

Patient Population / Client Focus\n

The LCSW will primarily work with patients in hospice care who are often grappling with terminal illnesses and the accompanying emotional distress. These patients may have complex psychosocial needs that require not only clinical expertise but also deep empathy and understanding. Families of patients will also be supported to help them navigate the emotional challenges they face during this critical time.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of 2 years of experience delivering psychotherapy and mental health services, particularly in end-of-life care or hospice settings.

Compensation and Benefits\n
