Work Environment

This position will operate in a home care setting, allowing for personalized, one-on-one engagement with patients in the comfort of their homes. Physical Therapists at Guided Grace provide innovative care to individuals who are recovering from surgeries, managing chronic illnesses, or experiencing mobility challenges related to age or injury. Key Responsibilities

• Evaluate patients to identify limitations in mobility and functional abilities, setting measurable goals that align with physician directives.
• Design and implement customized treatment plans, utilizing evidence-based practices to facilitate recovery and improve quality of life.
• Document clinical assessments, treatment procedures, and progress notes meticulously, ensuring compliance with healthcare standards and regulations.
• Supervise the activities of Physical Therapy Assistants (PTAs) and support staff, providing guidance and training to maximize patient outcomes.
• Engage regularly with interdisciplinary teams to assess and adjust treatment strategies based on patient progress, collaborating with physicians, nurses, and other healthcare providers.
• Lead patient education sessions, instilling knowledge and confidence in self-management techniques.
• Stay abreast of advancements in physical therapy techniques and technologies to provide the most effective care.

Qualifications

Compensation and Benefits
