Key Responsibilities\n

• Conduct thorough assessments of clients to determine their mental health needs and develop personalized treatment plans.

• Communicate effectively with supervisors, family members, and other relevant parties to share updates on clients' progress toward their therapeutic goals.

• Organize and maintain a schedule of appointments, home visits, and follow-ups to ensure ongoing support for your clients.

• Coordinate and execute home visits when necessary for in-home observations, data collection, and reviews, ensuring a comprehensive understanding of each client's needs.

• Implement therapeutic interventions and monitor their effectiveness, making necessary adjustments to treatment plans as required.

• Participate in interdepartmental Team Meetings to enhance collaboration across the organization concerning patient care strategies.
\n
• Prepare and complete monthly and quarterly progress notes that document client advancements and inform treatment direction.
\n
• Facilitate training and workshops for team members to ensure complete competency in program implementation and documentation standards.
\n
• Provide continued support through follow-up interviews and observations to ensure clients are engaged in their treatment journey.

• Maintain accurate and confidential records to ensure information accessibility for stakeholders, striking a balance between transparency and confidentiality.

• Act as a liaison with the broader mental health community, advocating for the needs of your clients and promoting community resources that can assist in their recovery.

• Contribute to the creation of training materials that will bolster the quality of programs provided to individuals.

Qualifications\n
