Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, patient observations, and medical record reviews to evaluate the emotional, social, and environmental influences affecting patient recovery.

• Provide empathetic counseling and support to patients and their families, addressing their adjustment issues and promoting emotional well-being.

• Collaborate with the rehabilitation team to formulate individualized treatment plans that consider each patient's psychosocial needs and goals.

• Assist patients in navigating community resources—such as support groups, vocational rehabilitation services, and housing assistance—to support their rehabilitation journey.

• Educate patients and families about post-rehabilitation services and available community resources to ensure a smooth transition into their next phases of life.

• Coordinate discharge planning efforts and facilitate successful transitions to home or other care settings.

Patient Population / Client Focus\n

Your work will primarily revolve around patients in a hospice setting, many of whom have experienced severe health conditions such as strokes, spinal cord injuries, or neurological disorders. You will address a spectrum of emotional and psychological challenges that accompany these medical conditions, ensuring that each patient receives tailored support aimed at enhancing their quality of life.
