Position Title: Co-Response Outreach Coordinator
Division: Community Health Services
Reports To: Director of Community Health Services
Job Summary:
The Co-Response Outreach Coordinator supports the Community by coordinating outreach activities, facilitating access to community resources, supporting program deliverables, and strengthening partnerships with community stakeholders. This position connects relatives and community members experiencing behavioral health, substance use, housing, crisis-related, or other social service needs to appropriate supports while assisting with documentation, data collection, event coordination, resource navigation, and communication with internal teams and external partners. The position helps promote coordinated, culturally responsive, and effective service delivery for individuals who may frequently utilize community, emergency, crisis, or justice-related resources.
Essential Functions:
The following functions are fundamental to the position and are required of all incumbents, with or without reasonable accommodation. These functions define the role and are not an exhaustive list of all duties.
- Coordinates and conducts Co-Response outreach activities with other GPTLHB program staff, community members, community partners, and other key stakeholders.
- Facilitates access to community resources, including behavioral health services, housing supports, food, clothing, transportation, vocational services, life-skills supports, crisis resources, and other social service resources as appropriate.
- Collaborates with external partners, including law enforcement, emergency response agencies, community-based organizations, behavioral health providers, housing resources, and other agencies as appropriate.
- Identifies and supports individuals or populations that frequently utilize Rapid City resources and programs, including emergency services and the Pennington County Jail.
- Conducts need assessments and provide emergency resource navigation for individuals experiencing substance use concerns, untreated behavioral health needs, serious housing barriers, or crisis-related needs.
- Works with agencies and service providers to support case coordination, information sharing, referrals, follow-up, and connection to wrap-around services.
- Engages proactively with Co-Response field teams and case managers to coordinate services and support continuity of care for community members in crisis.
- Coordinates and supports program activities, meetings, outreach events, conference calls, training activities, and community engagement opportunities related to Co-Response services.
- Assists with data collection, program tracking, reporting, and documentation of outreach contacts, referrals, partner engagement, service connections, and follow-up activities.
- Maintains accurate and complete program files, resource lists, referral information, meeting notes, outreach materials, and other records in accordance with program expectations and confidentiality requirements.
- Supports development and distribution of outreach, promotional, and educational materials, including flyers, resource information, meeting materials, and program communication tools.
- Supports formal agreements, purchase requests, invoices, supplies, travel arrangements, and other administrative coordination needs in accordance with funding requirements and GPTLHB policies/procedures.
- Fosters relationships with stakeholders to strengthen program collaborations, improve resource coordination, and reduce barriers to care for relatives and community members.
- Promotes effective and efficient use of community, clinical, crisis, and social service resources by coordinating with internal teams and external partners.
- Maintains and reports applicable statistics, outcomes, and program/service information as requested.
- Performs other duties as assigned that are consistent with the scope, purpose, and classification of the position.
Required Qualifications:
Bachelor’s degree in social work, human services, public health, behavioral health, criminal justice, psychology, sociology, or a related field and one (1) year of relevant experience; or associate degree in a related field and three (3) years of relevant experience; or high school diploma/GED and five (5) years of progressively responsible relevant experience.
Preferred Qualifications:
- Experience working with tribal communities, Co-Response teams, behavioral health providers, law enforcement partners, housing resources, crisis response programs, public health programs, or community-based outreach programs preferred.
- Ability to conduct needs assessments, identify barriers to care, and connect individuals to appropriate services and supports.
- Ability to communicate effectively in one-on-one, small groups, community, and partner settings.
- Ability to build professional working relationships with internal teams, external agencies, program partners, and community stakeholders.
- Ability to maintain a flexible work schedule, including evenings, weekends, local travel, overnight travel, or extended travel as needed.
- Work in office, clinical, community-based, and field/outreach settings.
- Sit, stand, walk, drive, lift/carry outreach materials, and travel locally or regionally as needed for meetings, outreach events, partner engagement, and service coordination.
- Communicate in person, by phone, and electronically with relatives, community members, team members, and external partners.
- Use a computer, keyboard, phone, and standard office equipment for documentation, reporting, scheduling, and communication.
- May encounter individuals experiencing crisis, behavioral health concerns, substance use concerns, housing instability, or other barriers to care.
- Maintain confidentiality, professional boundaries, safety awareness, and culturally responsive service practices in community-based settings.
Physical/Environmental Requirements:
- Ability to sit, stand, walk, bend, and move throughout the workday.
- Ability to drive locally or regionally for outreach, meetings, community engagement, and service coordination.
- Ability to lift and carry up to 25 pounds, including outreach materials, supplies, files, equipment, hygiene items, blankets, or boxed items.
- Ability to use a computer, keyboard, phone, and standard office equipment for documentation, reporting, scheduling, and communication.
- Ability to communicate in person, by phone, and electronically with relatives, community members, team members, and external partners.
- Ability to participate in outreach events, meetings, trainings, and field-based activities as needed.
- Work may be performed in office, clinical, community-based, and field/outreach settings.
- Local or regional travel may be required.
- Work may involve contact with individuals experiencing crisis, behavioral health concerns, substance use concerns, housing instability, or other barriers to care.
- Work may involve interaction with community partners, law enforcement, emergency services, service providers, and public/community settings.
- The position requires maintaining confidentiality, professional boundaries, safety awareness, and culturally responsive service practices in all work settings.
- The position may require flexibility in schedule, including evenings, weekends, or extended hours based on program or outreach needs.
