Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through patient interviews, observational methods, and medical record reviews. This includes evaluating emotional, social, and environmental factors that are critical to recovery.

• Collaborate with a multidisciplinary rehabilitation team, including therapists and physicians, to develop tailored treatment plans that consider the unique psychosocial needs of each patient.

• Assist patients in accessing community resources such as support groups, vocational rehabilitation, and housing assistance to facilitate their recovery.

• Offer education to patients and families regarding available community resources and post-rehabilitation services, ensuring they have the necessary tools for a successful transition.

• Engage in proactive discharge planning, supporting patients' transitions back to their homes or into other care settings to promote seamless continuity of care.
