Key Responsibilities\n

• Assist patients in navigating community resources, such as integration into support groups, vocational rehabilitation, and housing assistance, to foster continued personal growth outside the clinical setting.

• Educate patients and their families about available community resources and facilitate post-rehabilitation services that support holistic recovery.

• Engage actively in discharge planning, ensuring smooth transitions for patients as they return home or move to alternative care settings, reflecting continuity of care.

In this role, you will be pivotal in supporting a diverse range of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.
