Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observations, and the review of medical records to evaluate the emotional and social factors influencing patient recovery.

• Deliver individual and family counseling sessions aimed at understanding and addressing adjustment challenges and emotional well-being.

• Collaborate with an interdisciplinary rehabilitation team to formulate and implement individualized treatment plans that reflect the unique psychosocial needs of each patient.

• Facilitate access to community resources, including support groups, vocational rehabilitation programs, and housing assistance, to optimize patients’ transitions post-discharge.

• Provide educational resources to patients and their families regarding available community services and the continuum of care following rehabilitation.

• Participate actively in discharge planning to ensure seamless transitions to home or alternative care environments. This includes preparing patients and families for upcoming challenges and changes.

Your work will predominantly involve helping individuals who have experienced significant life changes due to various health conditions. We focus on supporting a diverse patient population, including those recovering from strokes, traumatic injuries, and chronic illnesses. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

Compensation and Benefits\n
