Key Responsibilities\n

• Participate in treatment-planning meetings to offer insights and receive feedback from multidisciplinary teams about patient progress and challenges.

• Identify and address resistance and conflict among clients, families, and healthcare team members, intervening as necessary to manage tensions and promote effective communication.
\n
• Compile and maintain accurate patient records, adhering to legal and ethical standards while documenting all assessments, treatment plans, interventions, and communication with external agencies.
\n
• Utilize the most current DSM diagnostic criteria and understand human growth and development to inform assessment and interventions for families and patients.
\n
• Apply evidence-based techniques such as behavior modification and assertiveness training to support clients in developing effective coping strategies.
\n
• Conduct comprehensive assessments to formulate personalized treatment plans that highlight the psychosocial strengths and needs of patients and families.
\n
• Facilitate educational and therapeutic groups, tracking patient goals and responses to enhance engagement and efficacy of therapy.

Patient Population / Client Focus\n

This role focuses on supporting patients facing life-limiting illnesses and their families, addressing both the emotional and social aspects of their experience. You will help guide them through their journey, providing vital resources and interventions tailored to their unique situations, while also embodying empathy and understanding of each family's cultural and spiritual needs.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline is required.

• Current and independent licensure as a Licensed Clinical Social Worker (LCSW) in California.

• Experience providing psychosocial support in a healthcare setting, particularly in hospice or palliative care, is highly desirable.

• Proficient understanding of the DSM diagnostic criteria and its application in clinical settings, particularly with pediatric and adolescent populations.

Preferred Experience\n

• Experience with electronic health record (EHR) systems, facilitating documentation and communication of patient care effectively.

• Training in grief counseling and family dynamics during end-of-life care.

• Comfort working in a remote setting, utilizing technology to maintain patient engagement and facilitate teamwork.

• Certification in Basic Life Support (BLS) and CPR.

Compensation and Benefits\n
