Work Environment

This role is entirely onsite, providing home care services directly to patients in their residences. As a Physical Therapist at Revive Motion Clinic, you will operate in a real-world setting, adapting therapy to the home environment while ensuring patient comfort and safety. The flexible, home-based work environment allows you to foster relationships with patients in their everyday settings, enhancing their rehabilitation journeys.

Key Responsibilities

Your daily activities will include:

• Designing individualized physical therapy treatment plans aimed to prevent disability, alleviate pain, and restore muscle function, all while ensuring adherence to the comprehensive plan of care.
• Supervising and mentoring Physical Therapy Assistants to guarantee effective and precise implementation of patient care plans.
• Conducting thorough evaluations of clients' physical abilities through diagnostic tests tailored to muscle, joint, and functional capabilities.
• Assessing home environments to identify obstacles and opportunities that could assist clients in achieving greater independence.
• Documenting clinical records—including initial evaluations, care plans, and progress notes—ensuring compliance with healthcare regulations and company policies.
• Collaborating closely with interdisciplinary teams during case conferences to refine treatment plans and share insights regarding patient progress.
• Ensuring timely submission of clinical and payroll documentation, keeping in line with branch policies.
• Instructing clients and their families on the effective use of therapy devices and techniques associated with their physical therapy programs.
• Providing rehabilitation services that are tailored to each client's unique circumstances and needs.

As a Physical Therapist, you will serve a diverse patient population, including individuals recovering after surgery, those managing chronic conditions, and seniors seeking to enhance their mobility and independence. Qualifications

The successful candidate will meet the following criteria:

• Graduation from an accredited physical therapy program.
• A minimum of one year's experience as a physical therapist, with a preference for acute care or homecare settings.
• Current and valid physical therapy license in good standing within New Jersey.
• Current CPR certification.
• A valid driver's license and automobile insurance.

Compensation and Benefits

Revive Motion Clinic offers a competitive salary ranging from USD 100,000 to 120,000 per year, commensurate with experience.
